Also known as the Affidavit of Small Succession, the Louisiana Small Estate Affidavit Form is provided by the Department of Revenue and is restricted to use within the state of Louisiana. The form legally records a deceased person’s estate and rightfully distributes it to the successors/heirs of the decedent.
Instructions to fill this form:
- Enter the full legal name of the decedent, and indicate his/her marital status. Also provide the detailed address of the decedent, his/her date of birth, social security number as well as name of parish in which the decedent resided.
- The affiant then accepts the declaration. The decedent has left no will, the value of the estate does not exceed $50,000, the estate contains no real estate and the decedent does not have any heirs apart from those mentioned in this affidavit.
- Space is provided to give details of four heirs as well as details of surviving spouse. If there are more heirs, attach a supplementary list. The details to be provided for the heirs are – full legal name, address, city, state, ZIP code, relation to decedent, social security number and signature of each heir. For surviving spouse, telephone number is also asked for.
- At the bottom, the left half is for office use only. The right half is for compulsory notarization by a Notary Public for the state of Louisiana. The form should be submitted back to the Department of Revenue.
