Instructions to Fill this Form

  • Begin by filling in the full legal name of the deceased person and also the county of Maine State in which he/she resided.
  • Fill in the full name of the affiant as well, along with his/her driving license number as identification proof. Also mention the dates between which the affiant knew the decedent.
  • Mention details of the decedent’s death – date, city, county, state of death and full residential address at the time of death.
  • Answer whether the decedent left a will – yes or no. If yes, answer whether the will has been admitted to probate. If yes, mention the place, cause number and date.
  • Give details of the decedent’s surviving widow/widower. If married more than once, give details of the former spouse as well.
  • Give names, addresses and other specified details of all surviving children of the decedent. Then give details of any deceased children. Then, give details of grandchildren of the decedent. If he/she had any adopted or step-children, provide their details too.
  • If the decedent left no surviving children, give details of surviving parents and siblings. If these don’t exist either, give details of other known relatives.
  • Mention the relationship between the affiant and decedent, and specify how long the two have known each other. The signature of the affiant is required followed by notarization of the affidavit.
